Connecting for Health Empowerment Using Health Information Technology to Transform Care in Multicultural Communities |
This first in a series of annual conferences hosted by the National Heath IT Collaborative for the Underserved will present "cutting edge" strategies that use health information technology (HIT) tools to advance health and eliminate disparities. While the focus of discussion is on multicultural underserved communities, presenters will offer solutions to help transform the nation's health care ecosystem. Our vision for this transformation is to ensure that all individuals in the United States have the tools, and use them, to attain and maintain their highest level of health and well-being. |
